RELEASE OF LIABILITY AND ASSUMPTION OF ALL RISKS

In attending the Sagittarius event being hosted by Promusement/Lisa Micklin on December 6, 2008 in Crestone, Colorado I am agreeing to the following:

  • I take personal responsibility for my actions, inactions and safety at all times on the premises of White Eagle Village, including both interior and exterior property.
  • I am aware of the availability or lack of availability of medical services on site.
  • I take personal responsibility for any injury (including fatality) that may occur during my participation in this event and release Promusement/Lisa Micklin and White Eagle Village from any liability whatsoever in regard to my injury or fatality. I understand that neither I nor my beneficiaries/heirs will be reimbursed by Promusement/Lisa Micklin or White Eagle Village for any expenses that may be incurred as a result of my injury or fatality.
  • I give my permission for emergency medical care to be administered to me, should a medical emergency arise during the event. Any expenses incurred will be my sole responsibility.
  • I take personal responsibility for all of my personal property on-site and I release Promusement/Lisa Micklin and White Eagle Village from any responsibility that may result in the damage, loss or theft of my personal property. I understand that I will not be reimbursed for any such damage, loss or theft.
  • I am aware that neither Promusement/Lisa Micklin nor White Eagle Village is responsible (financially or otherwise) for injury or property damage in the event of a property fire or other emergency that may damage my person or my property.
  • I am responsible for reading, understanding and complying with all posted signage at the event.
  • I agree to cooperate with any and all requests made to me by event security.
  • I understand that my entry fee is a revocable license that may be revoked by Promusement/Lisa Micklin, should I fail to comply with any of the above agreements.
  • RECOGNIZING THESE RISKS, I CHOOSE TO TAKE PART IN THE EVENT AND HEREBY RELEASE PROMUSEMENT/LISA MICKLIN AND WHITE EAGLE VILLAGE FROM ANY LIABILITY OR DAMAGES. I AGREE TO ALL APPLICABLE TERMS OF THIS AGREEMENT AND UNDERSTAND ITS CONTENTS AND SIGN IT WITH FULL KNOWLEDGE OF ITS SIGNIFICANCE. I AM OVER 18 YEARS OF AGE.
